- Valid photo ID (driver's license, passport, or state ID)

What happens
Your first visit, step by step
Walk In or Book Online
You can walk in during business hours without an appointment, or book an in-office visit online with the Book Online button. Online booking does not need a portal account.
Check In at the Front Desk
Arrive a few minutes early if it's your first visit. Our staff will get you checked in and verify your insurance.
Meet with Dr. Ucheagwu
Dr. Ucheagwu will review your health history, listen to your concerns, and develop a care plan tailored to you.
Prescriptions and Lab Work
If needed, prescriptions can be filled in our on-site pharmacy and bloodwork done without leaving the building.

We accept Medicare; Medicare Advantage plans from AARP/UnitedHealthcare (including Dual Complete), Wellcare (Fidelis Dual Align and Assist), Braven Health and Clover Health; NJ FamilyCare (Medicaid) through UnitedHealthcare Community Plan, Fidelis Care (Wellcare) and Aetna Better Health of New Jersey; Horizon Blue Cross Blue Shield of NJ (including OMNIA, Direct Access, Advantage EPO, HMO, POS, PPO, NJ Direct and NJ Educators); and Aetna, Cigna, UnitedHealthcare, Oxford, UMR, Ambetter and Oscar.
We do not accept Horizon NJ Health or Humana at this time. For Aetna Medicare Advantage, please ask our front desk.
